Family traditionally has been identified as parents and children living together as a single unit. In today’s social climate this is no longer the case. According to Wright & Leahey, (2013) “Family is who they say it is.” Today families are composed of same sex couples, “blended families” in which remarried couples bring children from previous relationships into a single family unit, single parent families have only one parent providing for a child. Grandparents are raising their grandchildren as a result of death, imprisonment, or abandonment. People who have no biologic ties but share an intense emotional attachment consider themselves family, such as members of a support group for alcoholism, ptsd, infant loss etc.

Recognizing these non-traditional families and understanding the intense emotional connection of the people in these family structures fosters cultural and holistic care which promotes optimal health outcomes. When people’s lives are interconnected in any way, anything that effects one member of the family unit effects everyone. A traumatic event that results in a reduction or loss of income will impact all members of the family because of the change of lifestyle. The family may have to make changes in housing, schools, entertainment etc. A disruption of any kind, positive or negative will impact all members of the unit. This is the foundation for the family system theory.

Family centered care is important if optimal health is to be achieved. For example; by involving adult children and/or grandchildren in the decision making process, after care and medication management of an elderly parent, there is a greater understanding of the disease process and a greater possibility of compliance and a reduction in the stress of recurrent problems due to mismanagement of the disease process.
