Evidence-based Practice and Application
Directions:
1.Identify two research articles that guided or will guide the development of your intervention. The articles can be original research, a meta-analysis, or published practice guidelines based on research. Examples of databases that can be used include PubMed, CINAHL, and the CDC Community Health Improvement Navigator website available at http://www.cdc.gov/CHInav/database/.
2.Briefly summarize the research and discuss how the findings guided or will guide your intervention. Include full citations for both articles at the end of the summaries. Remember to also cite the articles in-text.
3.Download the articles into a pdf or doc format and attach to Canvas along with the assignment. ALSO post a working URL and DOI number in the comments section of Bb. Points will be deducted if we cannot access your research articles when we are grading the papers.
This assignment should be written in a separate Word document using APA format which includes full reference citations, in-text citations, 12 pt Times New Roman font, double-spacing, and 1 inch margins. A title page and reference page are not necessary.

Population-focused Clinical Project Proposal
Directions: Write a proposal using the below headings.
- Introduction (1 -2paragraphs)
Introduce your clinical community site or public health issue that is your focus for the semester. You should provide a general description that includes the following introductory information:
•If you are working with an agency or organization, address the following:
- Identify the name of the agency/organization;
- Identify who the population is that is served by the agency/organization (i.e. a

problem);
1)the sociodemographics of the population (i.e. age, educational level, race/culture, gender, developmental characteristics such as elderly or school age) and
2)the characteristics or attributes of the population served by the agency that confer risk (i.e., low SES is associated with being uninsured which is a barrier to accessing needed healthcare);
- Provide a general description of services provided by the organization to the

population served and an approximate number of people to whom these
are provided.
•If you are focusing on a specific issue of concern (and not working with a specific
agency ororganization), address the following:
population in Lexington, KY; 25% of teenage girls in Lexington, KY; 10% of people with mental health disorders);
1)the sociodemographics of the affected population (i.e. age, educational level, race/culture, gender, developmental characteristics such as elderly or school age) and
2)the characteristics or attributes of the population affected by the problem
that confer risk (i.e., infants and children are particularly at risk
because…).
Provide background information on the general health status of the community or health-related factors associated with the issue of concern to which you have been assigned.
Supportive information MUST include data from:
- Your community assessment, windshield survey, and key informant interviews,
- Relevant statistics specific to your population. Look at data from the CDC website, the

- State the magnitude of the health problem or determinant of health;
- Discuss the health effects;
- Describe the actual or potential role of Public Health Nursing in intervening in response to this;
- Discuss actual or potential resources available to resolve the problem or to improve health outcomes associated with the problem;
- Identify protective factors in the community (such as a strong advocate on the city council or the presence of walkable neighborhoods).
health status of the population served by the agency; you may focus on one health
issue/problem or a determinant of health that you identified (i.e. lack of access to
health services or education)
i.e., If the focus of the agency to which you are assigned is to provide health services to Russian immigrants living in Fayette County, you might include the following:
Several health problems have been noted to particularly affect the Russian population living in the United States. According to data from the CDC (cite), lung cancer rates are higher in this population than in other U.S. ethnic groups. According to information from Mr. during the key informant interview (cite), lung cancer is also problematic among Russian immigrants living in Fayette County and is related to high rates of smoking among members of this community. This concern was supported by data collected during the windshield survey of the community served by the Clinic. As noted in the Windshield survey, convenient stores that sold both packs of cigarettes and individual cigarettes were located on nearly every corner. Also, groups of local residents were observed near the store smoking.
on health factors related to the issue.
i.e., If the focus of the issue of concern you are assigned to farm accidents in rural
areas in and surrounding Fayette County, you might include the following:
Agricultural accidents are among the highest causes of work-related injuries in the U.S. (cite CDC Wonder). Dr. Deborah Reed, whom we interviewed as our key informant, is an expert in injury reduction research in Kentucky (cite). According to Dr. Reed, Kentucky farmers are particularly susceptible to injury and death caused by tractor rollovers. During the windshield survey of farms in Fayette County, several incidents of tractors being used in rough and hilly terrain were noted.
- Health Problem (1-2 paragraphs)
- Intervention Proposal (2-3 paragraphs)
- Program Goal, Objectives, and Evaluation Methods
Based on the background information, identify a priority health-related problem and/or determinant of health related to the clinical population or associated with your issue of concern. Identified problems must be supported by the information you present in the background. Next, provide the following information about this:
- Describe the intervention you plan to implement at your site.
- Discuss how your intervention fills a gap/need for your clinical site.
- Describe what barriers should be anticipated when implementing the intervention in this community.
- Identify “community assets” that could be used as resources to support your intervention.
- Identify the main goal of your intervention and then identify at least 2 objectives of your intervention.
REMEMBER: These should address what the client/community/population will do, NOT what the nurse/student will do.
Example with the 4 parts of the educational objectives
80% of Wilmington High School students (who is expected to exhibit the behavior) will increase STD prevention knowledge (what the expected behavior is) by 10% after interactive lessons (conditions and qualifiers of the behavior) between August and December of 2005 (standards or time).
For each objective identified above, describe how you will determine/measure if that objective was met.
Example: Evaluation method for Objective 1: Wilmington High school students will complete a 5-question test before and after each lesson, which will demonstrate their ability to recall knowledge as well as evaluate comprehension by restating the material in their own words.
This assignment should be written in APA format which includes full reference citations,
in-text citations, 12 pt Times New Roman font, double-spacing, and 1 inch margins. A title page and reference page ARE REQUIRED.
