When is the traditional gold-standard research method of RCT not “enough”?

When is the traditional gold-standard research method of RCT not “enough”? What are the drawbacks and limitations of RCT? Is observational research the answer? In what ways can newer EHR-based methods of data analysis greatly increase the value and potential of observational research? Although the standards put forth by the Meaningful Use initiative are a good place to start, why are these initiatives not enough to advance best practices in all settings? What else is needed?
