Case Vignette:
A forty-year-old administrative assistant, named Felicia, was referred for an outpatient evaluation by her primary care physician with a chief complaint of always feeling “on edge.” She had never married or had children and lived alone.
Felicia had a live-in boyfriend of 10 years, but he abruptly ended the relationship and began dating a younger woman. Soon after that, Felicia began to agonize about making mistakes at work regarding routine tasks. She often felt tense and fatigued which was not typical for her. She also had difficulty focusing. She began to worry excessively over money and, to save money, she moved into a cheaper apartment in a less desirable neighborhood. No one seemed to be able to help her, and she worried about being “too much of a burden.”
Three months prior to the evaluation, Felicia began to avoid going out in the evenings, fearing that something bad would happen and she would not be able to summon help. More recently, she avoided going out in the daytime as well as she felt too vulnerable and “exposed” walking to the grocery store that was three blocks away. She was, however, able to figure out how to get her groceries delivered. If she was in her apartment, she could often relax and enjoy a good movie or book.
Felicia reported that she has always been a little bit of a worrier and stated she had cried inconsolably when her mother tried to drop her off at kindergarten. She saw a counselor at age 9, during her parents’ divorce, because her mother thought she was being too “clingy.” Felicia stated that she had never liked being alone and had constantly had a boyfriend since age 16. Before her breakup, she thought she had been doing “fine.” She reported she was successful at work, jogged regularly, maintained a good social network of friends, and had no complaints.
