1) Review your client session from Week 2

DON’T WORRY IF YOU DON’T HAVE IT SAVED, YOU CAN GO BACK INTO YOUR SUBMISSION TO VIEW IT AGAIN

2) Download and complete the SOAP Note Template & Instructions-1.doc  Download SOAP Note Template & Instructions-1.doc

3) Upload your completed document. Be sure that I am able read your note. If at all possible, type your SOAP note, rather than hand write it. In the field, you will typically be using electronic records. Good documentation is not useful if it cannot be read.
