The goal for this assessment is to succinctly summarize your community health findings and plans into an executive summary that leadership and stakeholders can easily understand and utilize when talking with other groups to raise support for the project.

There are many differences in cultural perspectives in relation to religious practices and beliefs. It is important for health care providers to understand these perceptions and beliefs for their impacts on illness and disease and also in response to approaches in education. Pérez and Luquis (2014) discuss the importance of understanding the role of spirituality in cultural competence, especially as it relates to vulnerable populations. Communicating health education must be responsive to the beliefs and attitudes in all areas of cultural differences, including those that are particular to religion and spirituality.

In many cultures, healers play a significant role in therapeutic treatment of illness and disease, and these practices must be taken into consideration when developing treatment plans that are culturally responsive. In order to apply culturally competent communication, the health care provider must be aware of the religious beliefs, symbols, practices, and rituals consistent with that culture.

Throughout this course you have explored a specific health need in a specific population or community. For this final assessment you will create a concise report that synthesizes your work throughout this course. The ability to create succinct communication that can be used by various stakeholders from diverse populations is a key skill for health care practitioners. This helps ensure transparency and that everyone has a fair opportunity to understand the care issues and care strategies being employed to help address health care needs and improve outcomes.

Community health assessments are a valuable approach to identifying and forming plans to address the health care needs of communities and populations within communities. Community health assessments take into account population health data, qualitative data from various community and professional stakeholders, epidemiological best practices, and cross-cultural collaboration strategies. To make the findings of a community health assessment useful to multiple parties, it is important to be able to succinctly communicate findings and plans to stakeholders. By being clear and concise in an executive summary, you make it easier for others to advocate for your projects and initiatives.

Scenario

Now that you have completed your research and planning, leadership has asked the project team to develop a brief executive summary that presents your key findings. The want to receive a cohesive document can be easily shared to illustrates the team’s intervention and health promotion plan to improve the quality of care and outcomes for the health need that was identified for your population of choice in your community.

Specifically, the leadership is interested in seeing:

  • A summary of community or state demographics, including ethnicity and levels of illness and health status.
  • An analysis of an area of concern for the health care need in the diverse population, including the following:
    • A summary of the National CLAS Standards in relation to the health concern.
    • An analysis of the findings from your interview with the health care professional.
  • An outline of an intervention and health promotion plan that will lead to improvements in the quality of care and outcomes related to the project’s identified health need for a target population.
    • Relevant and appropriate supporting evidence for the plan.
    • An assurance that the plan will contribute to cross-cultural collaboration.
  • An outline of strategies to help advocate for and implement the plan.

Instructions

For this assessment you will create an executive summary that succinctly summarizes your findings and work on the first three assessments in this course. The executive summary should also provide the audience with an idea of how they could advocate for action with regard to your findings, as well as some strategies to implement your intervention and health promotion plan.

The bullet points below correspond to the grading criteria in the scoring guide. Be sure that your submission addresses all of them. You may also want to read the Executive Summary: Community Health Assessment Scoring Guide to better understand how each grading criterion will be assessed.

In your executive summary, make sure to complete the following:

  • Integrate a variety of demographics to address the chosen health care need for a diverse community or state.
  • Summarize relevant community or state demographics connected to the chosen health care need.
  • Analyze findings from the health care organization interview as to its ability to implement strategies that align to the National CLAS Standards to address the identified health care need within the diverse population of the community or state.
  • Explain the key components of an intervention and health promotion plan and how the plan will lead to improvements in the quality of care and outcomes related to project’s identified health need for a target population.
  • Evaluate strategies to foster cross-cultural collaboration between diverse groups to ensure fair treatment of patients and equal access to care.
  • Identify strategies that stakeholders could use to advocate for and implement the intervention and health promotion plan.
  • Communicate in a concise and professional way that helps the audience to understand the community health assessment, as well as act as champions for awareness and implementation of the project.
  • Integrate relevant sources to support assertions, correctly formatting citations and references using current APA style.
Additional Requirements
  • Length of submission: 2–4 typed, double-spaced pages, not including the title and reference pages. Your executive summary should be thorough, but concise. It really should not be longer than four pages. No abstract is required for this assessment.
  • Number of references: Cite a minimum of 6–20 sources of scholarly or professional evidence. Resources should be no more than five years old, with the potential exception of the National CLAS Standards.
  • APA formatting: Use the APA Style Paper Tutorial [DOCX] to help you in writing and formatting your analysis.
