Acute Care Hospital Taking 500750 Proposed Projec
In collaboration with your approved course mentor, you will identify a specific evidence-based practice proposal topic for the capstone project. Consider the clinical environment in which you are currently working or have recently worked. The capstone project topic can be a clinical practice problem, an organizational issue, a quality improvement suggestion, a leadership initiative, or an educational need appropriate to your area of interest as well as your practice immersion (practicum) setting. Examples of the integration of community health, leadership, and an EBP can be found on the “Educational and Community-Based Programs” page of the Healthy People 2020 website.
Write a 500-750 word description of your proposed capstone project topic. Make sure to include the following:
- The problem, issue, suggestion, initiative, or educational need that will be the focus of the project
- The setting or context in which the problem, issue, suggestion, initiative, or educational need can be observed.
- A description providing a high level of detail regarding the problem, issue, suggestion, initiative, or educational need.
- Impact of the problem, issue, suggestion, initiative, or educational need on the work environment, the quality of care provided by staff, and patient outcomes.
- Significance of the problem, issue, suggestion, initiative, or educational need and its implications to nursing.
- A proposed solution to the identified project topic

I HAVE ATTACHED THE CITATIONS AND TRY TO STICK TO ONLY THOSE IF POSSIBLE. MY RESEARCH WILL TAKE PLACE IN AN ACUTE CARE HOSPITAL ON A TELEMETRY FLOOR WITH MANY ELDERLY PATIENTS THAT ARE IMMOBILE AND INCONTINENT. THE PROBLEM IS THAT THERE IS NOT ENOUGH STAFF TO TURN PATIENTS EVERY 2 HOURS AND CLEAN THEM UP AS OFTEN AS THEY NEED TO BE. COME UP WITH A PROPOSED SOLUTION SUCH AS PRESSURE REDISTRIBUTING SUPPORT MATTRESSES. HERE IS SOMETHING I WROTE ABOUT THIS ISSUE ALREADY.
Currently I am employed at an acute care hospital taking care of patients with cardiac problems. This floor faces many different problems which include falls, infection control, nurse shortage, skin breakdowns, and more. As a team we have protocols in place for most of our issues, but it is a work in progress. I would say our biggest clinical problem is skin breakdown. Our floor is a cardiac floor which means most patients are on a diuretic, and with the nurse and aide shortage, it may be hard for the nurses to turn their patients every 2 hours, let alone make it in time to take patients to the restroom. Elderly patients that get admitted to the hospital, can develop a pressure ulcer within their first week (Lyder, 2008). It is very important to prevent these ulcers, because it can lengthen their stay and effect our reimbursement rates. There are some problems that we cannot fix right away which is our shortage in nurses and aides. What we can possibly do is get more of the administration involved in our issues and make it known the reason why we are having so many skin breakdowns. Another way to implicate theses changes is to survey how many of these skin breakdowns were caused by us and how much this costs us at the end of the day. If administration realizes how much they are paying for this at the end, they might consider having more of a budget to try fixing this problem.
