SBAR

SBAR is acronym standing for Situation, Background, Assessment, and Recommendation. It is a communication tool often used in health care. It was originally created by the US military as a way to organize and quickly relay critical information. In health care, it is similarly used as a way to organize critical information that can be relayed in a clear and concise manner. The tool is a recommended practice by TeamSTEPPS which is an evidence-based framework advocated by AHRQ.

Situation is where you describe the situation or concern at hand in only a couple sentences. Background is where you provide a couple sentences of necessary historical context needed to understand the current situation. Assessment is where you provide a focused evaluation of the concern. It should not be more than a paragraph in length. Finally, Recommendation is where you provide your recommended course of action. Below is an example:

Situation: Hospital is experiencing a code black (utility failure) which has lead to a complete loss of the network.

Background: The loss of the network started approximately 0630 am by a motor vehicle accident two miles from the hospital campus which took out a Spectrum node and tore multiple fiberoptic cables.

Assessment: Loss of network has impacted EHR, wifi, phones, and now telemetry. Staff are also unable to log into the downtime computers. Cell service is now deteriorating as well. Spectrum is at accident site and advise that network will be down for several hours. Staff have already instituted a safety watch. IT Director on call has been notified .

Recommendation: Recommend that we institute Emergency Operations plan and open command center. Administrator on call is enroute to assume incident command.
