After reading the short section on clinical guidelines in the textbook and the articles on data and information systems, the Quality Improvement Committee asks you to collect some data on diabetic clients at your long-term care facility. You asked the electronic health record (EHR) database administrator to abstract some data from the EHR on all the clients with a diagnosis of diabetes. He gave you the attached Google Sheets file for your use. You can work with the file in Google Sheets or download the Google Sheets file into Excel. Your goal in Assignment 7 is to produce a table and a graph of baseline data so that you can track the progress as the Quality Improvement Committee implements their project.
For this discussion forum, look at the data in Google Sheets or Excel and decide if you’d like the database administrator to abstract more columns of data for each client. What additional columns would you like him to abstract? What is your rationale for the additional columns?
Then, In the initial post, state what tables or graphs would you like to present to the Quality Improvement Committee to show the current state of diabetic clients (baseline data) in the LTC. Describe the tables or graphs so that other students know what data you’d like to include. The goal is to create a picture of the current state of diabetes management at the facility.
Third, what baseline quality metrics can you calculate from the data? Why are these metrics important? Is there any additional data that you would need from the LTC for those metrics?

Clinical Decision Support Systems
Read the articles on clinical decision support systems in the Resources and Readings section. This should give you an idea of what a CDSS is and how it can be used in nursing. In a hospital, early warning systems on patient decline, alarms on telemetry monitors, and pop-up alerts on medications are examples of CDSS for nurses.
Describe in detail one positive and one negative example of a CDSS that you have encountered. Give your rationale for labeling the examples as positive or negative. How could the negative example be changed to be positive? The most frequent negative example is alarm fatigue, which could be changed through more selective alarms. Describe how you would change the alarms to be more selective if this is your negative example.
If you are don’t have an example from your own practice environment, how would a CDSS be useful in your practice environment? One example is a web-based resource library (a knowledge base) that is easily accessible at work. What would you include in list of web-based resources?
In your responses, expand on the ideas presented by others. How could the idea described in the initial post be expanded to be more useful or more available to nurses.
